Obstetrician gynecologists, internists and primary-care physicians refer new patients to UT Southwestern Medical Center, but patients may request an appointment for an examination without a doctor’s referral.
Patients and doctors have several scheduling options. They may request an appointment by using the online Request an Appointment Form or by calling 214-645-6455 or toll-free at 866-645-6455. They may also contact our offices directly.
Doctors may also use the online Physician Referral Form or call toll-free at 866-645-5455. They may also contact our offices directly.
For assistance with the management of complicated obstetrical cases, obstetricians are invited to call our Maternal-Fetal Medicine Transport Hotline, which provides direct 24-hour-a-day physician-to-physician access. It is also the number to call when arranging transport of patients to maternal-fetal medicine specialists at UT Southwestern University Hospital.
Making An Appointment
When making an appointment, the scheduling staff will ask for the following information:
- Patient’s name, address and phone number
- Patient’s date of birth
- Patient’s employer
- Patient’s insurance provider(s) and policy number(s), and
- The name, address and phone number of the referring physician, if patient is being referred.
Patients’ medical records are an important part of their first visits to UT Southwestern. Usually, the referring physicians will provide all reports, X-ray films and test results to patients or send them directly to UT Southwestern. Patients are asked to bring their medical records with them or verify that our staff has received the pertinent reports from their doctors.
New patients should plan to arrive one-half hour before their scheduled appointment to complete the necessary registration forms.
After-Hours and Emergency Care
Current patients with an urgent problem or needing to see a physician immediately should call UT Southwestern at 214-645-3838 to leave a page for our on-call physician.
If a patient has a problem that is life threatening, call 911 immediately and go to the nearest emergency room; UT Southwestern’s emergency room is located at UT Southwestern University Hospital at 5909 Harry Hines Blvd. (at Inwood Road).
If a patient is taken to another hospital’s emergency room, family members should inform the emergency room staff that the individual is a patient at UT Southwestern. After the patient’s condition is stabilized, the individual can be transferred to UT Southwestern. The emergency room physician will notify
UT Southwestern’s on-call doctor, who will determine the best approach for transfer.
